Kentucky State Police Investigate Breathitt County Fatal Collision

LOST CREEK, Ky. (March 14, 2025) – On Thursday, March 13, 2025, at 9:15 A.M, Kentucky State Police, Post 13, Hazard, received a call in reference to a two-vehicle collision on KY-476, in the Lost Creek community of Breathitt County.

Troopers, along with medical personnel responded to the scene. Initial investigation indicates that Carol A. Campbell, 78, of Hardshell, was operating a Grey Toyota Camry, northbound, on KY-476 when her vehicle crossed the center line into the southbound lane striking a White GMC gravel truck. Campbell was transported by ambulance to Kentucky River Medical Center where she succumbed to injuries sustained in the collision. She was pronounced deceased by the Breathitt County Coroners Office. The operator of the gravel truck sustained no injuries.

No seat belts were in use at the time of collision. Toxicology results and autopsy are pending at this time. This incident remains under investigation and is being reconstructed by Detective Scott Caudill.
