Kentucky State Police Investigate Fatal Collision in Lyon County

EDDYVILLE, Ky. (December 3, 2024) – Kentucky State Police (KSP) Post 1 is investigating a collision in Lyon County that claimed two lives on Monday afternoon. 

At 4:21pm CT on Monday December 2nd, KSP Post 1 dispatch received a report of a four-vehicle collision near the 42 mile marker on I-24 in Lyon County. Preliminary investigation shows that a 2000 Honda Passport, operated by Tiffany Anderson, 44 years old of Hopkinsville, KY was traveling westbound on I-24 with passengers Tara Rex, 52 years old of Hopkinsville, KY, James Simmons, 42 years old of Paducah, KY, James Sanders, 49 years old of Hopkinsville, KY and Brian Holt, 50 years old of Cadiz, KY. A 2020 International truck and trailer operated by Tomas Almaguer-Martinez, 54 years old of Guadalupe, Mexico was also traveling westbound on I-24 with a passenger, Antioco Garcia-Morales, 31 years old of Mexico City, Mexico. For reasons still under investigation, the Honda Passport operated by Ms. Anderson veared into the lane of travel of the truck, sideswiping that vehicle. The Honda Passport then crossed over the median into the eastbound lanes, colliding with two other vehicles traveling eastbound; a 2024 GMC Sierra operated by Tucker Heaton, 42 years old of Princeton, KY with passengers Jordan Thomas, 38 years old of Princeton, KY and Kerry Orange, 56 years old of Princeton, KY and a 2024 Freightliner truck and trailer operated by Dipen Tiwari, 26 years old of Pickerington, OH.  

Tara Rex and James Simmons were both ejected from the vehicle they were traveling in and pronounced deceased at the scene. Tiffany Anderson was transported to a local hospital for treatment of non-life threatening injuries and James Sanders was transported to an out of state hospital for serious injuries. No other injuries were reported. 

KSP was assisted on scene by the Kentucky Transportation Cabinet, Lyon County Sheriff’s Office, Lyon County EMS, Caldwell County EMS, Lyon County Coroner, Eddyville Police Department, Eddyville Fire Department and Kuttawa Fire Department. The investigation remains ongoing by Trooper Cody Jackson.
